    SWCSI complies fully with the Fair Credit Reporting Act guidelines. Clients must meet these guidelines before credits reports can be released.

    1. Click on the Download Forms below. Print these forms.
    2. Complete the Service Application, Service Agreement, Authorization for Release of Bank Information, FCRA Acknowledgement and Access Security Requirements.
    3. Fax the completed/signed forms along with a copy of your current mortgage brokers license to 408 360-0195.
    4. Upon verification of the signed forms and approval, we will contact you for technical support and training.
    SWCSI is required to verify all clients are bonafide business entities before activating your account. Please remember your need to have your borrowers' approval on file to generate credit reports on their behalf.
